Anterior commissure

The anterior commissure is a small band of white matter fibers that crosses the midline of the brain, connecting regions of the two cerebral hemispheres, particularly areas involved in smell and certain aspects of memory. In neonatal cranial ultrasound, this structure can be identified near the front of the third ventricle and helps sonographers orient themselves when evaluating the brain’s midline structures. While the anterior commissure itself is rarely the focus of clinical concern, being able to identify it is part of performing a thorough neonatal head ultrasound and can help in assessing midline brain abnormalities.
