Leiomyosarcoma is a cancerous tumor that develops from smooth muscle cells. While it can occur in many parts of the body, the uterus is one of the more common sites. Unlike benign leiomyomas, leiomyosarcomas grow aggressively and have the potential to spread to other organs. On ultrasound, they can be difficult to distinguish from benign fibroids, though features such as rapid growth, irregular borders, areas of necrosis, and increased internal blood flow on Doppler may raise suspicion. When a uterine mass shows concerning ultrasound features, further evaluation with MRI or biopsy is typically recommended.
