Retroperitoneal ultrasound evaluates the anatomical space behind the peritoneum, encompassing the aorta, inferior vena cava (IVC), kidneys, adrenal glands, pancreas, portions of the duodenum and colon, lymph nodes, and surrounding retroperitoneal soft tissues. Sonographers assess for retroperitoneal hematoma, lymphadenopathy, retroperitoneal fibrosis, primary retroperitoneal tumors (liposarcoma, leiomyosarcoma), and complications of conditions such as pancreatitis. The retroperitoneum is challenging to image due to depth and overlying bowel gas, requiring careful patient positioning and probe selection. This examination is commonly performed as part of a comprehensive abdominal sonographic survey.
