Testicular torsion is the twisting of the spermatic cord, causing ischemia of the testis and requiring emergent surgical detorsion within 6 hours to preserve viability. Color Doppler ultrasound is the imaging modality of choice, demonstrating absent or asymmetrically reduced intratesticular blood flow. The whirlpool sign — a twisted spermatic cord visualized in cross-section — is pathognomonic.
