Hepatocellular carcinoma is the most common primary liver malignancy, arising predominantly in the setting of liver cirrhosis. Ultrasound is recommended as a biannual surveillance tool in at-risk patients. CEUS demonstrates the hallmark arterial phase hyperenhancement followed by washout in the portal/late phase. A new nodule >1 cm in a cirrhotic liver with arterial hyperenhancement is diagnostic for HCC without biopsy.
