Splenomegaly is enlargement of the spleen beyond normal dimensions (length >13 cm on ultrasound). It is a common sonographic finding in portal hypertension, hematologic malignancies, infections, and storage diseases. Ultrasound also characterizes focal splenic lesions including cysts, hemangiomas, lymphoma, and metastases based on echogenicity, vascularity, and contrast enhancement patterns.
