The sonographic Murphy sign is elicited when the patient reports maximum tenderness as the ultrasound transducer is pressed directly over the visualized gallbladder. A positive sonographic Murphy sign, combined with gallstones and gallbladder wall thickening, has a high positive predictive value for acute cholecystitis. It is a key clinical finding assessed during every right upper quadrant ultrasound examination when cholecystitis is suspected.
