Shadowing vs. Enhancement

Posterior acoustic shadowing and posterior acoustic enhancement are opposite artifacts that provide important diagnostic information. Shadowing — a dark region deep to a structure — indicates strong reflection or absorption, typical of calcifications, gas, or dense tissue. Enhancement — increased brightness deep to a structure — indicates low attenuation, characteristic of fluid-filled structures like cysts or the bladder. Assessing posterior features is a fundamental part of lesion characterization on every ultrasound examination.
